Abstract

社區精神衛生照護是重要趨勢,護理學界與業界積極合作發展社區精神照護實務能力範疇;護理專業雖強調「以 人為本」的核心價值,在實踐層面仍需進一步發展護理照護之人本素質與能力。本研究目的旨在探討精神衛生護 理人員提供持續性社區護理照護時,應具備之人本要素。採現象學研究法,研究對象即為護理人員本人,以立意 取樣方式在某精神專科醫院,選取7位出院之精神分裂症患者,提供持續性護理社區照護。藉由家訪、電訪及門診 所提供照護過程,做成記錄與內容分析,探討護理人員之社區護理能力之人本要素。研究結果發現人本要素可含 三項主軸:(一)「傳遞溫暖」,包含個案感受被關懷、接納之概念;(二)「促進自覺」,包含引導探索內在資源、 接受自我之概念;(三)「共舞調適」,包含形成夥伴關係、促使發展因應調適之概念。本研究成果在實務應用方 面可作為護理人員在培訓社區精神衛生護理能力人本要素參考。 Community psychiatric mental health care has been viewed as an important future trend, and the academic and clinical nursing experts have made great efforts to develop the community psychiatric mental health nursing scope. We always lay stress on the humanistic core value, however, the humanistic essence and competency need development for better improvement. The purpose of this study is to explore the humanistic elements of community psychiatric mental health nursing competency. In this paper, we adopted a phenomenological qualitative research method, and the nurse as the research subject; in addition, purposive sampling, seven schizophrenic outpatients were recruited from an psychiatric hospital for continuous community psychiatric care, including home visit, telephone call and outpatient unit care. Data analyses of nursing process were used and made written records according to the continuous community care necessary to those outpatients. The results demonstrates three main humanistic elements, which are (1) “offering transmitting warmth”: including patient’s feeling of being cared and being accepted; (2) “facilitating promoting self-awareness”: including intrinsic resources exploration and self acceptance; (3) collaborative adaptation: including partnership and coping-adaptation. The outcome is believed able to be served as a training reference for improving the humanistic competency of the community psychiatric mental health nurse.
