A 40 year old female presented with productive cough, breathlessness and intermittent chest pain of 2 months duration. A diagnosis of bilateral primary leiomyosarcoma of lung was made on the basis of CT guided core biopsy on one side of lung and open biopsy on the other side followed by immunohistochemistry. The differential diagnosis, pathophysiology, diagnosis and line of management are presented here.
