Porous Titanium to accelerate bone regeneration for oral implantology

Abstract

INTRODUCTION: Alloplastic grafts used in combination with allogenic or xenogenic bone was demonstrated to have good results in term of new bone formation and the long term implants success rate [1]. In the present study, a novel sinus lift technique is presented, using porous titanium in combination with PRP (Platelet Rich Plasma) [2], demonstrating a significant development of new bone and minimizing any complications in the event of Schenederian membrane perforation. METHODS: Porous titanium (Tigran TM PTG) with 80% air and 20% pure titanium) was used in a clinical case with maxillary posterior edentulism. The mixture of porous titanium with plasma rich in growth factors was used as grafting material, and evaluated after 6 months with CBCT. In the second surgery stage, the new bone was collected with a trephine bur and submitted to histological analyses, prior to implants placement into the grafted area. RESULTS: Fig. 1 reveals the porosity of the grafting material used in this case, along with the radiographic image recorded by CBCT. The examination demonstrated that porous titanium was well integrated into the natural bone reaching a density of 629 Hounsfield units after 6 months. Histological examination of the bone collected revealed processes of bone proliferation in full decoiling-polymorph bone trabeculae from very thin to thick-majority formed by primary bone (nonlamellar). As presented in Fig. 2, general aspect is a typical bone proliferation in a relative advanced stage. DISCUSSION & CONCLUSIONS: In the present study it was demonstrated that healing period upon sinus lift surgical procedure can be reduced by combining alloplastic material (porous titanium) with PRGF (Plasma Rich in Growth Factor).
