Abstract

提供癌末病患人工營養與水分時常被認為是維持生命、舒適及減輕痛苦有關的治療方 式,造成了病患、家屬及醫護人員普遍傾向於人工水分及人工營養的使用,而人工營 養與水分卻常帶給癌末病患許多的不舒適,如肢體腫脹、全身水腫,甚至呼吸喘和呼 吸困難等。而醫護人員是最具影響病患和家屬是否選擇人工營養與水分之人物。所以 引發筆者想以營養代謝的角度探索癌末病患對於人工營養與水分的需求,本文主要以 正常的營養代謝、癌末病患之營養代謝以及提供癌末病患人工營養與水分的適切性等 三方面加以討論。期望醫護人員對於此主題有較清楚的瞭解,當未來面對癌末病患人 工營養與水分之抉擇時,能適時協助病患及家屬做出最適當的決定,以提高癌末病患 的生活品質為終旨。 Provision of artificial nutrition and hydration ( ANH) in terminal cancer patients is usually recognized to be a kind of treatment with increasing, comfort, and relieving suffering. That pushes patients, families and medical professionals to use the supply of nutrition and hydration. ANH often cause untoward effects in terminal cancer patients such as the limbs swelling, edema, even dyspnoea. Medical professionals are most important persons influencing patients and families to choose ANH. This paper uses the stand point of nutritional metabolism to explore terminal cancer patients' need of ANH. The paper major discusses with normal metabolism, terminal cancer patients' metabolism, and the appropriateness of providing ANH in terminal cancer patients. These discussions was hoped to be helpful for medical professionals to have critical thinking toward the use of ANH in terminal cancer patients. Further more, to be able to face the issue about supplying ANH in terminal cancer patients and assist patients, families to make appropriate decision to achieve the best quality of life in terminal cancer patients. ; 護理學系 ; 醫學院 ; 期刊論文
